How do I embed the Appt Request form onto my clinic's website?

You will receive a snippet of html code during your onboarding call, which needs to be added to your clinic's website code.

  1. From your website editor, create a new html page where you’d like the Appointment request form to appear. You can use your page template with your website’s header, footer and other components as usual.

    1. Note: if you're using a website builder (e.g. Wix), you can add a html code block anywhere on the page where you'd like the form to appear. 

  2. Copy and paste the provided code anywhere in the HTML where you would like to show the appointment form.

    1. Note that this will always be inside the <body> tag.

    2. Note that <DOMAIN> in the embed code needs to be replaced with your practice's website.
